The Disease Management Guide is a health care management tool for both disease and wellness care. It is designed to be used by a broad variety of health care providers. Most challenging is that it is designed to provide information in an efficient format so that it may be used as a reference at the point-of-care for the busy rural primary care physician.

A great deal of information has already been published in multiple sources about disease management, and the core essentials will be repeated here as well. The emphasis here will be on providing information that:

• Maximizes the providers ability to remain fiscally viable despite the financially challenging health care environment.
